The guy who ran the place was Lloyd Hickey. He ran a few different places in the Sacramento area. His sidekick was Sandra "Sunny" Cantrel. (Not his wife, but business partner in some of the ventures)

It says he ran 3 different Clubs in the last picture, but I think he had at least 4 or 5. He also ran a 49 Club in Sacramento.
I am missing the $5 chip from my collection. I would trade/buy if you have interest.
