My list is always way behind my purchases, but I use excel (i am an advanced user and can easily pivot or filter the data). I catalog the following:

Casino Name
City
Area
State
Denom
Mold
Type (House, Bac, Etc)
Body Color
Inlay Shape or HS
Inlay/HS Color
Insert Type
Insert Color
Size
Other Notes
Condition

Acquisition Info:
Seller
Cost
Date

For photo's i upload the photos to a website like Photobucket and set up folders (Vegas, Tahoe, Reno, Etc). that way i can view them on my phone with an app or any pc.
