My GUESS is that the chip was not completely pressed the first time, looks like the press had incomplete and/or uneven pressure the first time. The operator noticed when the chip was removed and put it back in for a second go, but reversed it (put it in "upside-down"?). That is the only way I can think of to have the crossed canes. Even at that it would have taken a lot of luck to line up the mold so precisely for the second press to hit almost perfectly in the same position for the hats.
I eagerly await someone who really knows the process to reply and solve the mystery!
