2nd opinion, same answer grin

Look closely, the misstrike is beneath the centered one. If this was under normal production that would not have happened.
Now look even more closely. Left hand stamp is a lighter impression, done at an earlier time. And, some of the foil that should be in the center circle from the left stamp has been removed.

I hypothesise

1. Chip originally in machine for hot stamping. Gets stuck and is stamped off center. Operator culls it to the floor.
2. Next morning operator turns on press and tests for heat. Picks up that chip off the floor, scrapes center area clean and tests. Stamp is good so he culls it to the floor again and starts work.

Had the stamp not been good he would probably have flipped it and tried again.

Most would say not an error
I tend to see it as an error chip...
