This is the one that is listed on the Chip Guide (CG047931) that I added a few months back.

The front has a clean Hot Stamp but the back side was never stamped. Here are some pictures and link to the Auction. I also have about 900+ items on Ebay so if you get time take a look and if don't find anything there take a look at my website chip store for more chips.
