I would like to modify your description of what happened a little. For the design to be raised on the chip it must be recessed into the die. What part of the die could become dislodged without it causing a larger raised area on the chip. In error coins there is an error known as a 'filled die' When grease and dirt clog one of the letters (or numbers) on the coin die it causes at letter (or number) to look like it is missing. I think that is what happened here. As to the second area it might have been damage from the same dirt or grease casing damage on the other part of the die. If it matches the filled area they call that a 'dropped letter' or 'dropped number' error
