I bought this chip from Doug Saito in a recent auction. Inadvertently tossed out the auction material and now cannot remember the MC chip attribution. Checked w/doug and he sent me a second set of the catalogue but the #126 number page was missing. Before I annoy him again for the I.D. thought someone on the board would recognize the chip.
