Someone asked about this one awhile back and I was there tonight and got one. Not sure how long they have been on the tables, but one matching it's description was approved by NGC back on 2-27-12.

It matches the inlay of the 3rd issue $1 that came out around 2009, with a smaller $5 on the side and a spelled out FIVE DOLLARS at the bottom of the small inlay.
