When I lived in Michigan, there were two insurers who had a reputation for denying claims unless the policyholder filed suit. They were well known among attorneys, who would not bother negotiating pre-suit. Those insurers also complained that so many dollars went to defending lawsuits (and they sought tort reform as a result), as if that was the general public's fault.
