This all comes from Shaffer's program (Collector's Assistant). One of the features is the report writer that will allow you to build a report. In this case I selected the image and text i wanted and it will create the report. Then I just use PrimoPDF to print the report to a PDF file.
What I have done afterwards (and the one i keep online is probably not the updated copy) is to use a program called JPDF bookmarks (http://sourceforge.net/projects/jpdfbookmarks/) to create a list of bookmarks for a table of contents. So when I transfer it to the iPad its easy at the convention to jump to either a letter or a casino. Bookmarks are a feature of some of the more expensive Acrobat and paid PDF creators however I am using free software to do the same thing.
For a couple of years I printed the book and that worked great however with the PDF on the iPad it works out real well.
