OK, so it isn't a crest and seal chip -- it's die-cut. I just got these chips in the mail today. Never saw this before. The sample record cards (reproduced below) show they were ordered and shipped from Cincinnati to the US Playing Card Co. New York office in 1919. We don't know who used them.

Look at these two pictures -- they are all the SA chips I have. The second picture is a close-up:

Here is a close-up of the bottom 3 chips:

I think these chips are pretty rare. I don't recall seeing them on eBay recently. I've had the blue one for a while. The 4 SA white chips in row 2 are normal inlay-wise. The three on the bottom (row #4) I can't figure out. They are perfectly smooth; you can't feel the inlay (or anything) with your finger nail on the smooth surface. Yet, if you tilt it, you can see the shiny image of the SA! Like a ghost. Haunted chip? Magical powers? The lone white chip (row #3) has the normal SA inlay on one side, and the ghost inlay on the other side!
I's sell one of the normal white ones (row #2) for $50 delivered and one of the three ghost ones in row #4 for $50 delivered. Return priv. PayPal OK.
