Found this on eBay last week. It a cast medal, plated, and looks to be two uniface tokens back to back. Size is about 2" in diameter. I don't see it in the Nevada promo tokens book.

Any ideas? Could this have been the medallion the dealers wore on a chain? At Caesars Palace, they are gold, but maybe Tahoe used silver?
