Charles, I obtained a few of these chips about 5 years ago from a woman who's husband had dealt cards at a few places in and around the Bedford, Indiana, area. All of the chips obtained had the monogrammed 'G' on the obverse side with the $5 Horseshoe hotstamp reverse side (Like shown) except for one chip. One of the chips had the $5 Horseshoe hotstamp on both the obverse & reverse sides (No 'G' monogram on the obverse side). I have the chip as unattributed.
