Do you use a customs form to Canada, UK?

Say you have a light (2-13 ounces), small, 3/4" thick kraft envelope or bubble envelope with several chips going to Canada or Europe or Australia. Do you bother to put a customs form on it (like the one pictured below)? Ever get into trouble not using a form?

If you do:
¶ call it a "gift" or "other"?
¶ write casino chips or poker chips or game pieces or game tokens?
¶ say -0- or $5 or nominal amount or full amount?
