These were made by Rene Nezhoda. Rene put on a chip show at Palace Station and had these chips and plaque available for purchase. As Don indicates, I believe that your admission was good for one chip, but the others had to be bought. I recall getting the set of chips at the time, but passed on the plaque. However, I did manage to get a plaque at the 2011 Convention for $10.

If memory serves me right, Rene also worked with Palace Station to get a live chip issued, either for the show, or at around the same time. I can't locate a scan right now, but I recall the chip having a large "MM" on one side. (Anyone have a scan?)

Also sharing the common reverse was this additional chip that Rene handed out to helpers and others (I assume Dealers) at the show.
