Here are links to 2 different versions of the 2011 OTY Awards Nomination Forms.

Please note that both forms are .pdf and will require Adobe Reader, which is already loaded to most computers, but if not, it is available as a free download from Adobe.com.

The first version is strictly a .pdf form that is intended to be printer and filled-in by hand. While it is double-sided (appearinmg as 2 pages) it can be printed as 2 pages or, if your printer has a duplex mode, you can print it out as a 2-sided document. The key thing here is that it is a form that is intended to be printed and then filled-in by hand.

This next version of the same form is also a .pdf document, but it can be filled-in electronically (on your computer) and when completed, it can then be printed (as above) and mailed via snail mail or FAXed. PLEASE NOTE! Filling-in this on-line version will NOT keep your changes when you click Save or Quit, so print it when given the chance. If you wish to email it, then scan as a .jpg and email that .jpg file to: OTY@CCGTCC.com
