It is difficult to know for sure for there are Cuban collectors that like to maintain their privacy, but I believe this is the only known example of a crest and seal from the Military Club, Campo Columbia, Marianao (just west of Havana), Cuba.
Campo Columbia was built by the US Military in the 1890s. When the US left Cuba in 1909, the Circulo Militar ("Military Club") became a social club for the Cuban military.
The chip was delivered in 1915, which is also the oldest known Cuban crest & seal.
Of note: Charles Lindbergh was feted at the Military Club in 1928.
To make the chip even rarer, it has the hard-to-find mounting pin holes!
