I'd say the result of a broken coining die. Pieces break off in the high stress areas. A lot of the BudJones coin-inlay chip exhibit this kind of defect. I wouldn't call it a variety, but some collectors do collect them all. Kind of in the category of poorly-registered color in the printing on a center inlay.
