Quite possibly the chip was made as a sample, or co-opted into a sample after one side was stamped for Club Cabazon and for reasons we don't know the order was never completed. Rather than toss the chip, a manufacturer logo was added to one side and then it become a salesmen's sample.
While Dennis of TRKing may know the answer, it seems we do not.
Anyway, when I showed two of these on TheChipBoard, I was contacted by a collector who wanted to buy one, and I sold it. The other one is now also offered on a Best Offer over $24.99 basis. (The chip shown in this scan.)
