Here is a match I found recently on eBay. It's a front striker and offers 35c hamburgers [g], so it's probably from the 60's or early 70's. The only listing I see in TGT is for a similarly named business that has only been there a few years. I'm not sure if the "21" on the match really means a 21 table game; it could mean slot games with blackjack on them, as they were introduced about the same time that video poker was.

Anyone have more history on this bar?
