Dr. Gary Potter from Eastern Kentucky University has published the subject book through the History Press. The book is basically a minor update of an article that has been available for years on the Net at:

You can either pay $19.95 for it at Amazon or download it for free from the above. While there has been some update, the authors have not corrected the many errors that are in the original text.
