Luke; I have a similar chip with five GOLD inserts listed/illustrated in my Puerto Rico Catalog that has been attributed as being from SAN JUAN HOTEL ... but I don't remember who provided me with that attribution... or how accurate it is. The San Jeronimo hot stamp chips have "San Jeronimo" spelled out fully.
