Wondering if anyone has any info on the patch, or the helmet, seen in the picture?

The patch is attached to the back side of the helmet - The helmet was accompanied by a 11 x 14" photo of a F-4B Phantom II on the flight deck of the USS Forrestal. The tail letters of the jet date it to around 1961-1966, just before the Forrestal went on tour in the Gulf of Tonkin and had the big fire.
