A lot of computers come with a "Recovery Partition" which is used if your operating system (WinXP) becomes corrupted. It would have the installation files for WinXP, drivers for any peripherals that are installed, and any additional software that was installed when you bought the computer. Sounds like that is what your D: drive is, only 5% free space sounds about right so I would leave it alone.
