This cover has very bad fading on the inside! 
In fact, I thought it was blank until I saw Mark's scan of a similar cover. (Page 118) Note however that the front uses blabk ink and not purple. The faded inside is the same as your lower-left version, with the "DUdley 2-7171" telephone number.
