Almost all tables at strip casinos are now $25, particularly on weekends and holidays. Mid-week you may find $10 or $15. A few downscale places like Sahara still have $5 BJ. Haven't seen a $2 table in years.

The main casino at Caesars Palace now has $50 to $200 minimums on weekends. Lower minimums still available in the newer sections but no $5 games that I've noticed.
