I was not familiar with the $5 chip ... but the $1 chip illustrated below has been published in my Puerto Rico Catalog since 1999. It was purchased from Albert Rollins at a CC & GTCC Convention around that time ... Albert told me that the CH chip was from Caribe Hilton in Puerto Rico. I'll defer to Michael Siskin's research.
