Dennis; You will get your form that is included with the club magazine that is mailed to all regular and Life Members. Since Associate Members do not receive a copy of the magazine ... they get their forms separately, via First Class Mail. If LM's have not opted for First Class Mail delivery of the club magazine ... it is entirely possible that Associate Members might get their forms before Life Members.

One thing is 100% certain ... trolls and NM's will not receive anything.
