The revised edition of The Gaming Table shows an Annex Club Card Room in Calexico, California, but doesn't list a particular mold.

No listing in Eastman's California guide. Calexico, California listing on John Kallman's site, but no pictures. Ed Hertel's book shows there's also an Annex Club in Texas, but it has a flower mold. Ed also shows a Hotel Annex in Montana, but on an incused mold. My vote goes to California, since it's a large crown.
