I like to use, generally, 2" x 2" plastic flips in 20-slot plastic Cowen pages. Instead of using those white paper insets (that go into the flips), I have been using "permanent" gummed labels, as shown in the picture below. With a fine point pen, I can write three lines of information on the labels. If you use labels, don't get the "removable" labels that are widely sold in stores; the gum will harden over time and the labels will fall off. Instead use the "permanent" type. Avery Dennison, for one, makes them. They come in various sizes. I like stock item #7087 -- 2" x 7/16." Also good is #7500 (1-3/4" x 3/8"). A big advantage of the labels over the cardboard flips and paper inserts is that you can see the back and edges of the chips. (If you use 20-slot Cowen pages and cardboard flips, it is hard to see the edges of the chip, though you can with 12-slot pages, but I like 20- or 30-slot pages.)
