The 4 insert version is most likely a prototype. I had about 90,000 Crystal Park $1 chips and only found about 4 of those so there are not many around. I found similiar versions of the $2 (only 2) and $3 (4 or so) chips too but no $5, $10, $20, $25, $100, $500, $1000 or $5000. They seem to be almost a laminate overlay.
