EXCELLENT question. The important thing is the chips themselves. The form is by no means required (especially for color variations). However, it would be nice if I had the year that each chip was produced.

The information gathered from the forms will be used to describe the chips when I get a webpage going displaying the chips online (with the owners permission of course).
