I use the Avery 8167 (1/2 X 1 3/4). I have my collection in an Excel spreadsheet (an Access database would work as well) and print sheets of labels using the Mail Merge and Publisher software. The trick is to print the information you want on the label on the top half and then fold the label around the bottom of the flip. The label acts as a seal for the flip. I don't staple the flips and just leave the two sides open so that it is easy to insert and remove the chip. This works fine if you put the flips in either a box or an album sheet. For the box version you need to print the label upside down and print on the bottom, not the top, of the label.
Somewhat awkward process but the best I can devise.
