I have two examples of this chip for sale. One is well nigh perfect, the other has a tiny mark.

Attributed to Ernie Primm's Club Reno by James C, argued and disputed by almost everyone else. Could be Ernie Primm's Club Gardena, or Embassy Palace Club Gardena (also owned by Primm). Could even have been taken from CA to Reno and used.
