The form that was included in my previous post was not intended for member use at this point in time, but merely to bring it to the attention of those who may have missed it last year.

Please wait until the form is distributed in the 4th quarter issue of the club magazine. (BTW: The THIRD Quarter magazine has not been mailed yet). The return date for next years HOF considerations will be January 31, 2005.
