unfortunately, the club site is hosted on a Unix/Apache system that does not run ASP pages, so I wouldn't be able to write a survey system to run on that site. Perhaps another member who knows PHP/Java/CGI could build one for that server.

For me to help, the club would have to move to a Windows-based server or allow me to host it on my server.
