Couple of questions about Harvey Fuller.

Have his notes ever been published? I know they were recorded on the backs of keno cards and matchcovers, and that a published book entitled Harvey Fuller's Index of Nevada Gaming Establishments exists. However, I've never seen a copy of the Index, so I don't know whether it's just a detailed listing, or whether some of the "notes" might be included.

Also, is the Fuller Index likely to list some smaller establishments that might not be included in The Chip Rack or The Gaming Table? I'm thinking of bars that only had a couple of slot machines and no live games, or only had live games for a short time.
