The difference between entrance fee and parking fee is that, I would think, the parking fee goes to the casino or the operator of the garage or valet service for the costs involved in providing that service or facility. The cost of insurance, maintenance, security etc. Some casinos will comp all or part of this for regular customers as is the case when I go to Hollywood in Aurora, IL. I can get FREE self parking in their garage or reduced Valet service, $3 instead of $5.
The entry fee is for everyone who enters regardless of whether they arrived by car, foot, train or whatever, and in the Illinois case, I understand it goes to THE STATE. Again some casinos are appearently absorbing this cost for their regular repetitive customers, (club card holders). If you arrive in a car with 4 friends with you you pay 1 parking fee but all 5 of you also pay an entry fee. In the Hollywood case, assuming you do not have a Marquee or Screen Test(?)card you would thus pay $2 for parking (self park) and $5 for entry. Totally $7 or $5 for Valet plus $5 to get in and maybe tip of $2 for the Valet bringing it to $12 just to get in. Again, unless any or all of you have club cards.
