Heres what I know.
Chipco thinks they sent it to a distributor in Europe. They are still looking. If it was sent to a foreign distributor, then only the distributors name would be on the order.
I have one left. $10 if anyone wants it.
It has the Chipco Ad on the back. Many of us feel these are legitimate chip variations. Greg lists them in the Chipguide.
Chipco does not put their logo on the back of real chips anymore.
