I don't know the answer to either question. There are tons of slots only indian casinos that don't appear in the chipguide. There are also lots of Indian bingo operations that might or might not be considered casinos.

Some of the earliest Indian casinos were operated on the sly with no formal compacts or licensing. Some even in the Chief's garage!
