The chip on the right is the error, while the one on the left shows what should have been. Please ignore the darkness of the CIC's, as the brightness control artificially darkened them to bring out the complete lack of red inserts.

It was given to me by a poker dealer at the Colorado Belle, who said that he plucked it from the table on opening day (of the poker room) and has held it since giving it to me last year.
