What molds have records?

Of the molds with records... which one's can I send off to get copies of the records?

Is there a list with names, addresses and prices for the individuals with the records?

P.S. Any other info regarding mold verification is appreciated... e.g., do all of the chips listed in Ed Hertel's book have mold records.
