Here are the two different chips (courtesy of Paul Hegge). Evidently, the printing for the $25 chip ended up on a $5 inlay.

Any ideas as to how this can be explained?

Josh-What is the condition of your chip? The image on my computer makes it look worn. Does it look like it was in-play?
