I'll take a shot at this one. IF every item made has the same mistake, it is not a limited unique error, i.e., all of those items known to exist are the same.
Chip issues are miniscule when compared to stamps or coins or cards when looking at numbers issued. Later issues of a corrected mistake will not be identical to the original (minus the mistake) in the case of casino chips, the chip manufacturers make other subtle changes when making a new run of what is supposed to be the same (similar to) chip.
