I had a similar question a while back. I use the Cowens 12 pocket sheets and use the 2.5x2.5 cardboard flip. I right a bunch of information on the cardboard and put it in the pocket.

For a binder I have looked a number of them and stick with the zippered binders. To date I have tried 5 or 6 different ones and today's favorite is the "American Studio" ones you can get from Office Depot. They are 2" rings for $10 and are stiff enough to stand up with only partial pages (which make the pages top heavy).

I use different colors for different types of chips (blue = AC, green = LV, red = Horseshoe, Lt. Blue = California... etc). then I can grab the book I want pretty easily.

I looked at custom making my own binders and went so far as to get a quote. I looked at the D-ring 2.5" binder with slipcase and custom printing. They priced out at $18 each for 500. I thought about it but decided to stick with off-the-shelf products for now.
