The #'s have the following meaning:

Top left: denomination
Top right: chip pack #
Middle: casino name
Bottom left: City, State
Bottom right: grade

ICG uses a 1-70 grading scale - the same as coins.
Chips that grade XF40-45 are slightly used nice examples
Chips that grade AU50-53-55-58 are barely used and almost new
Chips that grade 60 & up are new chips with circulation.
