I don't know how you did it, but I can tell you how I did one once.

I scanned the token which results in a bad looking hologram, but a good looking token. Then, I took a digital photo of the token with a light aimed at the right angle to make the hologram show up good. Then, using photoshop, I cut the hologram center out of the digital photograph and sized it to fit inside the scan I took. Came out real nice. Is that how you did it?
