As I said above, I have been happy with using the stiff plastic 2" x 2" "safety flips" and a 2" x 7/16" "permanent" Avery stock #7087 gummed label, as shown in the picture below. With a fine point ball point pen I can write a lot of info about the chip (I could also write more with another label on the reverse side). I like this set-up over the cardboard flips for a number of reasons -- one being that you can see the entire chip including the sides/edges. Important: don't use the ordinary gummed labels; the gum dries out (or whatever) over time, and in two or so years will fall off the flip.
