Not a Nevada chip, so no TCR #.
The chip you show is pictured in Howdy's "Nevada" book -- pg. 27, bottom.
He calls this style of chip "Engraved Coin Inlay Plastic Check" or ECIP for short.
No mention of who manufactured it. He does mention that the indention in the center of the metal inlay appears to be the spot that the 2 sides of the discs are joined (like a weld spot, I guess.)
While it's a generic chip in a sense, I haven't seen too many of them. Good chip to add to you "chip type" collection.
