I never heard of taxes being withheld or a W2 issued just for cashing in chips, Allen. They ask if you got it at 21 or craps, and if you say yes, there's no tax implications as these are low odds games. If you say you won it at roulette, the story might be different (30:1 payoffs), but I don't know that from personal experience.
